MEMBERSHIP APPLICATION

Membership Fees are $50 per year; plus a $20 Application Fee.​

Please Note:

• Complete the form below, and click "SEND". You will be mailed a Paper Application.

• Mail the Application along with a check for $70.

• All Applicants must attend a personal Interview at the Club.

• You will be contacted by email to confirm your date/time slot.

• If you decide at the Interview that you do not wish to join, the Membership Fee will be returned to you.

Do NOT submit an Application if you:

• Are under the age of 21.

• Are under felony indictment, or any other crime punishable for a term exceeding one year.

• Have been convicted in any court of a felony, or any other crime punishable by imprisonment for a term exceeding one year even if you received a shorter sentence or probation.

• Are a fugitive from justice.

• Are an unlawful user of, or addicted to, marijuana, or any depressant, stimulant, or narcotic drug or any controlled substance. Warning: The use or possession of marijuana remains unlawful under Federal law regardless of whether it has been legalized or decriminalized for medicinal or recreational purposes in the state where you reside.

• Have ever been adjudicated mentally defective or ever been committed to a mental institution (which includes having been adjudicated incompetent to manage your own affairs).

• Have been discharged from the Armed Forces under dishonorable conditions.

• Are an alien illegally or unlawfully in the United States.

• Have ever renounced your United States citizenship.

• Are subject to a court order restraining you from harassing, stalking, or threatening your child or partner; or child of such partner.

• Have ever been convicted in any court of a misdemeanor crime of domestic violence.
